The Cambridge aided loudness profile (CALP): item choices and data for unaided listening by young and older normal-hearing listeners and older hearing-impaired listeners

Objective To develop an improved version of the profile of aided loudness (PAL), intended for assessment of the appropriateness of the loudness of everyday sounds. Design Initially, 16 participants with a range of ages and degrees of hearing loss indicated whether they encountered each situation described in the PAL and how specific they considered the description to be. Based on the responses, most situations from the PAL were eliminated and new situations were introduced, giving the Cambridge Aided Loudness Profile (CALP). The CALP was administered to 80 young and 22 older participants with normal hearing, who rated the loudness of each situation and satisfaction with this loudness (as for the original PAL). Satisfaction was strongly negatively correlated with loudness, suggesting that satisfaction was largely based on loudness. The CALP was then administered to 32 new young normal-hearing participants and 49 older participants with hearing loss, most of whom used hearing aids, who rated loudness and the appropriateness of loudness. Results Some situations were rated as loud but appropriate in loudness, indicating that the CALP can distinguish these aspects. Conclusions The CALP questions were understood by all participants. The CALP may be useful for assessing the appropriateness of loudness.

Immediate Effects of (Simulated) Age-Related Hearing Loss on Cognitive Processing and Performance for the Backward-Digit-Span Task.

The recall of auditorily presented sequences of digits in reverse order (also known as the Backward Digit Span, BDS) is considered to reflect a person's information storage and processing abilities which have been linked to speech-in-noise intelligibility. However, especially in aging research and audiology, persons who are administered the BDS task are often affected by hearing loss (HL). If uncorrected, HL can have immediate assessment-format-related effects on cognitive-test performance and can result, in the long term, in neuroplastic changes impacting cognitive functioning. In the present study, an impairment-simulation approach, mimicking mild-to-moderate age-related HLs typical for persons aged 65, 75, and 85 years, was used in 19 young normal-hearing participants to evaluate the impact of HL on cognitive performance and the cognitive processes probed by the BDS task. Participants completed the BDS task in several listening conditions, as well as several commonly used visual tests of short-term and working memory. The results indicated that BDS performance was impaired by a simulated HL representing that of persons aged 75 years and above. In the normal-hearing condition, BDS performance correlated positively with both performance on tests of short-term memory and performance on tests of working memory. In the listening condition simulating moderate HL (as experienced by the average 85-year-old person), BDS performance only correlated with performance on working-memory tests. In conclusion, simulated (and, by extrapolation, actual) age-related HL negatively affects cognitive-test performance and may change the composition of the cognitive processes associated with the completion of a cognitive task.

Model-based hearing-enhancement strategies for cochlear synaptopathy pathologies

It is well known that ageing and noise exposure are important causes of sensorineural hearing loss, and can result in damage of the outer hair cells or other structures of the inner ear, including synaptic damage to the auditory nerve (AN), i.e., cochlear synaptopathy (CS). Despite the suspected high prevalence of CS among people with self-reported hearing difficulties but seemingly normal hearing, conventional hearing-aid algorithms do not compensate for the functional deficits associated with CS. Here, we present and evaluate a number of auditory signal-processing strategies designed to maximally restore AN coding for listeners with CS pathologies. We evaluated our algorithms in subjects with and without suspected age-related CS to assess whether physiological and behavioural markers associated with CS can be improved. Our data show that after applying our algorithms, envelope-following responses and perceptual amplitude-modulation sensitivity were consistently enhanced in both young and older listeners. Speech-in-noise intelligibility showed small improvements after processing but mostly for young normal-hearing participants, with median improvements of up to 8.3%. Since our hearing-enhancement strategies were designed to optimally drive the AN fibres, they were able to improve temporal-envelope processing for listeners both with and without suspected CS. Our proposed algorithms can be rapidly executed and can thus extend the application range of current hearing aids and hearables, while leaving sound amplification unaffected.

The Concurrent OLSA Test: A Method for Speech Recognition in Multi-talker Situations at Fixed SNR.

A multi-talker paradigm is introduced that uses different attentional processes to adjust speech-recognition scores with the goal of conducting measurements at high signal-to-noise ratios (SNR). The basic idea is to simulate a group conversation with three talkers. Talkers alternately speak sentences of the German matrix test OLSA. Each time a sentence begins with the name “Kerstin” (call sign), the participant is addressed and instructed to repeat the last words of all sentences from that talker, until another talker begins a sentence with “Kerstin”. The alternation of the talkers is implemented with an adjustable overlap time that causes an overlap between the call sign “Kerstin” and the target words to be repeated. Thus, the two tasks of detecting “Kerstin” and repeating target words are to be done at the same time. The paradigm was tested with 22 young normal-hearing participants (YNH) for three overlap times (0.6 s, 0.8 s, 1.0 s). Results for these overlap times show significant differences, with median target word recognition scores of 88%, 82%, and 77%, respectively (including call-sign and dual-task effects). A comparison of the dual task with the corresponding single tasks suggests that the observed effects reflect an increased cognitive load.

Speech intelligibility and talker gender classification with noise-vocoded and tone-vocoded speech.

Vocoded speech provides less spectral information than natural, unprocessed speech, negatively affecting listener performance on speech intelligibility and talker gender classification tasks. In this study, young normal-hearing participants listened to noise-vocoded and tone-vocoded (i.e., sinewave-vocoded) sentences containing 1, 2, 4, 8, 16, or 32 channels, as well as non-vocoded sentences, and reported the words heard as well as the gender of the talker. Overall, performance was significantly better with tone-vocoded than noise-vocoded speech for both tasks. Within the talker gender classification task, biases in performance were observed for lower numbers of channels, especially when using the noise carrier.

Clinical Importance of Binaural Information: Extending Auditory Assessment in Clinical Populations Using a Portable Testing Platform.

PurposeThe purpose of this study is to use variability on tests of basic auditory processing to allow identification of those tests that could be used clinically to describe functional hearing ability beyond the pure-tone audiogram and clinical speech-in-noise tests.MethodPsychoacoustic tests implemented using the Portable Automated Rapid Testing system on a calibrated iPad were evaluated for nine young normal-hearing participants (M age = 21.3, SD = 2.5) and seven hearing-impaired participants (M age = 64.9, SD = 13.5). Participants completed 10 psychoacoustic subtests in a quiet room. Correlational analyses were used to compare performance on the psychoacoustic test battery with performance on a clinical speech-in-noise test and with the 4-frequency pure-tone average (4FreqPTA).ResultsSpectral processing ability was highly correlated with 4FreqPTA, and temporal processing ability showed minimal variability across the hearing-impaired group. Tests involving binaural processing captured variability across hearing-impaired listeners not associated with 4FreqPTA or speech-in-noise performance.ConclusionsTests that capture the ability to use binaural cues may add information to what current clinical protocols reveal about patients with auditory complaints. Further testing with a larger sample size is needed to confirm the need for binaural measurements and to develop normative data for clinical settings.

Development and evaluation of video recordings for the OLSA matrix sentence test

Objective The aim was to create and validate an audiovisual version of the German matrix sentence test (MST), which uses the existing audio-only speech material. Design Video recordings were recorded and dubbed with the audio of the existing German MST. The current study evaluates the MST in conditions including audio and visual modalities, speech in quiet and noise, and open and closed-set response formats. Sample One female talker recorded repetitions of the German MST sentences. Twenty-eight young normal-hearing participants completed the evaluation study. Results The audiovisual benefit in quiet was 7.0 dB in sound pressure level (SPL). In noise, the audiovisual benefit was 4.9 dB in signal-to-noise ratio (SNR). Speechreading scores ranged from 0% to 84% speech reception in visual-only sentences (mean = 50%). Audiovisual speech reception thresholds (SRTs) had a larger standard deviation than audio-only SRTs. Audiovisual SRTs improved successively with increasing number of lists performed. The final video recordings are openly available. Conclusions The video material achieved similar results as the literature in terms of gross speech intelligibility, despite the inherent asynchronies of dubbing. Due to ceiling effects, adaptive procedures targeting 80% intelligibility should be used. At least one or two training lists should be performed.

AVATAR Assesses Speech Understanding and Multitask Costs in Ecologically Relevant Listening Situations.

There is a high need among clinicians and researchers for an ecologically valid measure of auditory functioning and listening effort. Therefore, we developed AVATAR: an "Audiovisual True-to-life Assessment of Auditory Rehabilitation" which takes important characteristics of real-life listening situations into account, such as multimodal speech presentation, spatial separation of sound sources and multitasking. As such, AVATAR aims to assess both auditory functioning and the amount of allocated processing resources during listening in a realistic yet controllable way. In the present study, we evaluated AVATAR and investigated whether speech understanding in noise and multitask costs during realistic listening environments changed with increasing task complexity. Thirty-five young normal-hearing participants performed different task combinations of an auditory-visual speech-in-noise task and three secondary tasks on both auditory localization and visual short-term memory in a simulated restaurant environment. Tasks were combined in increasing complexity and multitask costs on the secondary tasks were investigated as an estimate of the amount of cognitive resources allocated during listening and multitasking. In addition to behavioral measures of auditory functioning and effort, working memory capacity and self-reported hearing difficulties were established using a reading span test and a questionnaire on daily hearing abilities. Whereas performance on the speech-in-noise task was not affected by task complexity, multitask costs on one of the secondary tasks became significantly larger with increasing task complexity. Working memory capacity correlated significantly with multitask costs, but no association was observed between behavioral outcome measures and self-reported hearing abilities or effort. AVATAR proved to be a promising model to assess speech intelligibility and auditory localization abilities and to gauge the amount of processing resources during effortful listening in ecologically relevant multitasking situations by means of multitask costs. In contrast with current clinical measures of auditory functioning, results showed that listening and multitasking in challenging listening environments can require a considerable amount of processing resources, even for young normal-hearing adults. Furthermore, the allocation of resources increased in more demanding listening situations. These findings open avenues for a more realistic assessment of auditory functioning and individually tuned auditory rehabilitation for individuals of different age and hearing profiles.

The long-term learning effect related to the repeated use of the Finnish matrix sentence test and the Finnish digit triplet test

Objectives: To assess are there learning-related improvements in the speech reception thresholds (SRTs) for the Finnish matrix sentence test (FMST) and the Finnish digit triplet test (FDTT) in repeated use over 12 months. Design: Test sessions were scheduled at 0, 1, 3, 6 and 12 months, and each session included five FMST measurements and four FDTT measurements. The within-session and inter-session improvements in SRTs were analysed with a linear mixed model. Study sample: Fifteen young normal-hearing participants. Results: Statistically significant mean improvements of 2.0 dB SNR and 1.2 dB SNR were detected for the FMST and the FDTT, respectively, over the 12-month follow-up period. For the FMST, majority of the improvement occurred during the first two test sessions. For the FDTT, statistically significant differences were detected only in comparison to the first test session and to the first test measurement of every session over the 12-month follow-up. Conclusions: Repeated use of the FMST led to significant learning-related improvements, but the improvements appeared to plateau by the third test session. For the FDTT, the overall improvements were smaller, but a significant within-session difference between the first and consecutive FDTT measurements persisted throughout the test sessions.

Noise-induced enhancement of envelope following responses in normal-hearing adults.

Measures of signal-in-noise neural encoding may improve understanding of the hearing-in-noise difficulties experienced by many individuals in everyday life. Usually noise results in weaker envelope following responses (EFRs); however, some studies demonstrate EFR enhancements. This experiment tested whether noise-induced enhancements in EFRs are demonstrated with simple 500- and 1000-Hz pure tones amplitude modulated at 110 Hz. Most of the 12 young normal-hearing participants demonstrated enhanced encoding of the 110-Hz fundamental in a noise background compared to quiet; in contrast, responses at the harmonics were decreased in noise relative to quiet conditions. Possible mechanisms of such an enhancement are discussed.

Speech intelligibility of virtual humans

The speech intelligibility benefit of visual speech cues during oral communication is well-established. Therefore, an ecologically valid approach of auditory assessment should include the processing of both auditory and visual speech cues. This study describes the development and evaluation of a virtual human speaker designed to present speech auditory-visually. A male and female virtual human speaker were created and evaluated in two experiments: a visual-only speech reading test of words and sentences and an auditory-visual speech intelligibility sentence test. A group of five hearing, skilled speech reading adults participated in the speech reading test whereas a group of young normal-hearing participants (N = 35) was recruited for the intelligibility test. Skilled speech readers correctly identified 57 to 67% of the words and sentences uttered by the virtual speakers. The presence of the virtual speaker improved the speech intelligibility of sentences in noise by 1.5 to 2 dB. These results demonstrate the potential applicability of virtual humans in future auditory-visual speech assessment paradigms.

Effect of harmonic rank on sequential sound segregation

The ability to segregate sounds from different sound sources is thought to depend on the perceptual salience of differences between the sounds, such as differences in frequency or fundamental frequency (F0). F0 discrimination of complex tones is better for tones with low harmonics than for tones that only contain high harmonics, suggesting greater pitch salience for the former. This leads to the expectation that the sequential stream segregation (streaming) of complex tones should be better for tones with low harmonics than for tones with only high harmonics. However, the results of previous studies are conflicting about whether this is the case. The goals of this study were to determine the effect of harmonic rank on streaming and to establish whether streaming is related to F0 discrimination. Thirteen young normal-hearing participants were tested. Streaming was assessed for pure tones and complex tones containing harmonics with various ranks using sequences of ABA triplets, where A and B differed in frequency or in F0. The participants were asked to try to hear two streams and to indicate when they heard one and when they heard two streams. F0 discrimination was measured for the same tones that were used as A tones in the streaming experiment. Both streaming and F0 discrimination worsened significantly with increasing harmonic rank. There was a significant relationship between streaming and F0 discrimination, indicating that good F0 discrimination is associated with good streaming. This supports the idea that the extent of stream segregation depends on the salience of the perceptual difference between successive sounds.

Age-Related Differences in the Processing of Temporal Envelope and Spectral Cues in a Speech Segment.

As people age, they experience reduced temporal processing abilities. This results in poorer ability to understand speech, particularly for degraded input signals. Cochlear implants (CIs) convey speech information via the temporal envelopes of a spectrally degraded input signal. Because there is an increasing number of older CI users, there is a need to understand how temporal processing changes with age. Therefore, the goal of this study was to quantify age-related reduction in temporal processing abilities when attempting to discriminate words based on temporal envelope information from spectrally degraded signals. Younger normal-hearing (YNH) and older normal-hearing (ONH) participants were presented a continuum of speech tokens that varied in silence duration between phonemes (0 to 60 ms in 10-ms steps), and were asked to identify whether the stimulus was perceived more as the word "dish" or "ditch." Stimuli were vocoded using tonal carriers. The number of channels (1, 2, 4, 8, 16, and unprocessed) and temporal envelope low-pass filter cutoff frequency (50 and 400 Hz) were systematically varied. For the unprocessed conditions, the YNH participants perceived the word ditch for smaller silence durations than the ONH participants, indicating that aging affects temporal processing abilities. There was no difference in performance between the unprocessed and 16-channel, 400-Hz vocoded stimuli. Decreasing the number of spectral channels caused decreased ability to distinguish dish and ditch. Decreasing the envelope cutoff frequency also caused decreased ability to distinguish dish and ditch. The overall pattern of results revealed that reductions in spectral and temporal information had a relatively larger effect on the ONH participants compared with the YNH participants. Aging reduces the ability to utilize brief temporal cues in speech segments. Reducing spectral information-as occurs in a channel vocoder and in CI speech processing strategies-forces participants to use temporal envelope information; however, older participants are less capable of utilizing this information. These results suggest that providing as much spectral and temporal speech information as possible would benefit older CI users relatively more than younger CI users. In addition, the present findings help set expectations of clinical outcomes for speech understanding performance by adult CI users as a function of age.

Hearing Loss and Age-Induced Changes in the Central Auditory System Measured by the P3 Response to Small Changes in Frequency.

Recent behavioral studies have suggested that individuals with sloping audiograms exhibit localized improvements in frequency discrimination in the frequency region near the drop in hearing. Auditory-evoked potentials may provide evidence of such cortical plasticity and reorganization of frequency maps. The objective of this study was to evaluate electrophysiological evidence of cortical plasticity related to cortical frequency representation and discrimination abilities in older individuals with high-frequency sensorineural hearing loss (SNHL). It was hypothesized that the P3 response in this group would show evidence of physiological reorganization of frequency maps and enhanced neural representation at the edge of their high-frequency loss due to their restricted SNHL. The P3 auditory event-related potential in response to small frequency changes was recorded in a repeated measures design using an oddball paradigm that presented upward and downward frequency changes of 2%, 5%, and 20% to three groups of listeners. P3 recordings from a group of seven older individuals with a restricted sloping hearing loss >1000 or 2000 Hz was compared to two control groups of younger (n = 7) and older (n = 7) individuals with normal hearing/borderline normal hearing through 4000 Hz. The auditory P3 was recorded using an oddball paradigm (80%/20%) with the standard tone at the highest frequency of normal hearing in the hearing-impaired participants, also known as the edge frequency (EF). EFs were either 1000 or 2000 Hz for all participants. The target tones represented upward and downward frequency changes of 2%, 5%, and 20% from the standard tones of either 1000 or 2000 Hz. Waveforms were recorded using a two-channel clinical-evoked potential system. Latency and amplitude of the P300 peak were analyzed across groups for the three frequency conditions using repeated measures analysis of variance. The results of this study suggest that the P3 response can be elicited by frequency changes as small as 2-5%. P3 responses at the EF of hearing loss were present and larger in amplitude for more participants with a sloping hearing loss compared to age-matched normal-hearing peers tested at the same frequencies. As a result, the older participants with sloping hearing losses had P3 responses more similar to the younger normal-hearing participants than their age-matched peers with normal hearing. These preliminary results partially support the idea of enhanced cortical representation of frequency at the EF of localized SNHL in older adults that is not purely due to age.

The Effect of Decreased Audibility on MMSE Performance: A Measure Commonly Used for Diagnosing Dementia.

Hearing loss and dementia are both prevalent in late adulthood. The most common test used to determine cognitive status in late adulthood, the Mini-Mental State Examination (MMSE), is presented face to face, usually in the context of the physician's office in the presence of background noise. Despite the problems of hearing loss and cognitive problems in late life, there is an absence of evidence linking hearing-related deficits to performance on the MMSE and dementia diagnoses. This study examined the effect of decreased audibility on performance on the MMSE. A between-subjects design was implemented. Participants were randomly assigned to one of five degrees of simulated hearing loss conditions and were blinded to condition assignment. One hundred and twenty-five young normal-hearing participants were randomized into five conditions of varying degrees of simulated hearing loss. Performance on the MMSE was scored and cognitive status was categorized based on the scores. Analysis of variance with conditions as a between-subjects factor was conducted with post hoc multiple comparisons to determine the effect of audibility on performance. Reduced audibility significantly affected performance on the MMSE in a sample of young adults, resulting in greater apparent cognitive deficits as audibility decreased. Apparent cognitive deficits based on MMSE scores obtained in test conditions in which audibility is reduced could result in incorrectly identified cognitive loss if clinicians are not alert to hearing loss when patients are evaluated. Furthermore, health care providers should be cautious when using family report of cognitive impairment to diagnose dementia without accounting for hearing loss because the impression of family members may be based on misinterpretation of the effects of hearing loss.

Perceptual asymmetry induced by the auditory continuity illusion.

The challenges of daily communication require listeners to integrate both independent and complementary auditory information to form holistic auditory scenes. As part of this process listeners are thought to fill in missing information to create continuous perceptual streams, even when parts of messages are masked or obscured. One example of this filling-in process-the auditory continuity illusion-has been studied primarily using stimuli presented in isolation, leaving it unclear whether the illusion occurs in more complex situations with higher perceptual and attentional demands. In this study, young normal-hearing participants listened for long target tones, either real or illusory, in "clouds" of shorter masking tone and noise bursts with pseudorandom spectrotemporal locations. Patterns of detection suggest that illusory targets are salient within mixtures, although they do not produce the same level of performance as the real targets. The results suggest that the continuity illusion occurs in the presence of competing sounds and can be used to aid in the detection of partially obscured objects within complex auditory scenes.

Intelligibility of whispered speech in stationary and modulated noise maskers

This study investigated the role of natural periodic temporal fine structure in helping listeners take advantage of temporal valleys in amplitude-modulated masking noise when listening to speech. Young normal-hearing participants listened to natural, whispered, and/or vocoded nonsense sentences in a variety of masking conditions. Whispering alters normal waveform temporal fine structure dramatically but, unlike vocoding, does not degrade spectral details created by vocal tract resonances. The improvement in intelligibility, or masking release, due to introducing 16-Hz square-wave amplitude modulations in an otherwise steady speech-spectrum noise was reduced substantially with vocoded sentences relative to natural speech, but was not reduced for whispered sentences. In contrast to natural speech, masking release for whispered sentences was observed even at positive signal-to-noise ratios. Whispered speech has a different short-term amplitude distribution relative to natural speech, and this appeared to explain the robust masking release for whispered speech at high signal-to-noise ratios. Recognition of whispered speech was not disproportionately affected by unpredictable modulations created by a speech-envelope modulated noise masker. Overall, the presence or absence of periodic temporal fine structure did not have a major influence on the degree of benefit obtained from imposing temporal fluctuations on a noise masker.

Apparent auditory source width insensitivity in older hearing-impaired individuals

Previous studies have shown a loss in the precision of horizontal localization responses of older hearing-impaired (HI) individuals, along with potentially poorer neural representations of sound-source location. These deficits could be the result or corollary of greater difficulties in discriminating spatial images, and the insensitivity to punctate sound sources. This hypothesis was tested in three headphone-presentation experiments varying interaural coherence (IC), the cue most associated with apparent auditory source width. First, thresholds for differences in IC were measured for a broad sampling of participants. Older HI participants were significantly worse at discriminating IC across reference values than younger normal-hearing participants. These results are consistent with senescent increases in temporal jitter. Performance decreased with age, a finding corroborated in a second discrimination experiment using a separate group of participants matched for hearing loss. This group also completed a third, visual experiment, with both a cross-mapping task where they drew the size of the sound they heard and the identification task where they chose the image that best corresponded to what they heard. The results from the visual tasks indicate that older HI individuals do not hear punctate images and are relatively insensitive to changes in width based on IC.

Effect of Speech Material on the Benefit of Temporal Fine Structure Information in Speech for Young Normal-Hearing and Older Hearing-Impaired Participants

The purpose of this study was to investigate the influence of the type of speech material on the benefit obtained from temporal fine structure (TFS) information in speech for young normal-hearing (YNH) and older hearing-impaired (OHI) participants. The design was based on the work of . They measured the speech reception thresholds for a target talker in a background talker as a function of the frequency range over which TFS information was available. The signal was split into 32 channels, each with a bandwidth equal to the equivalent rectangular bandwidth of the "normal" auditory filter at the same center frequency. Above a cutoff (CO) channel, channels were vocoded and contained only temporal envelope information. Channels up to and including CO were not processed. Hopkins et al. found that, as CO was increased, speech reception thresholds decreased more for normal-hearing participants than for participants with cochlear hearing loss, suggesting that the latter were less able to use TFS information. We used the same design, but compared results when the target speech materials were open-set sentences, as used by Hopkins et al., and when they were more predictable sentences with a closed word set (Danish Dantale 2). With the open-set material, YNH listeners benefited more from TFS information than OHI listeners, replicating . For the YNH participants, the benefit of adding TFS was greater for the open-set material than for the closed-set material, while no difference in TFS benefit across speech materials was found for the OHI participants. The choice of speech material is important when assessing the benefit of TFS. Several factors may facilitate recognition in the absence of TFS cues, including small set size, predictable temporal structure of the target speech, and contextual effects. We speculate that TFS information is useful for reducing informational masking, by providing cues for the perceptual segregation of the target and background. When the target speech is highly predictable, informational masking may be minimal, rendering TFS cues unnecessary.

Neural Coding of Sound Intensity and Loudness in the Human Auditory System

Inter-individual differences in loudness sensation of 45 young normal-hearing participants were employed to investigate how and at what stage of the auditory pathway perceived loudness, the perceptual correlate of sound intensity, is transformed into neural activation. Loudness sensation was assessed by categorical loudness scaling, a psychoacoustical scaling procedure, whereas neural activation in the auditory cortex, inferior colliculi, and medial geniculate bodies was investigated with functional magnetic resonance imaging (fMRI). We observed an almost linear increase of perceived loudness and percent signal change from baseline (PSC) in all examined stages of the upper auditory pathway. Across individuals, the slope of the underlying growth function for perceived loudness was significantly correlated with the slope of the growth function for the PSC in the auditory cortex, but not in subcortical structures. In conclusion, the fMRI correlate of neural activity in the auditory cortex as measured by the blood oxygen level-dependent effect appears to be more a linear reflection of subjective loudness sensation rather than a display of physical sound pressure level, as measured using a sound-level meter.
